Luhn algorithm confusing

1. Which numbers have to be doubled? 

2. Which  numbers we keep as they are?
3. When we count from reverse order ? 

It was not clear from the video. 

The instructor writes too quickly.

4. What if card numbers have various digits lengths ?  (not 16) ? How to count otherwise ? 

Please explain it longer. Thanks.
